- Ask Clinical Question (Adult) For medical queries about yourself or another adult (aged 16 and over).
- Ask Clinical Question (Under 16 yrs) For medical queries about a child or young person. A parent or guardian should complete this form.
- Ask an Admin Question For non-medical queries such as forms, documents, fit notes, or general enquiries.
All submissions are reviewed by our team and passed to the most appropriate clinician or staff member.
We aim to respond within 2 working days (excluding weekends and bank holidays).
